Clinical safety focuses on identifying, evaluating and managing risks affecting participants during a clinical trial or development programme. Pharmacovigilance is a broader discipline covering the detection, assessment, understanding and prevention of adverse effects and other medicine-related problems across a product’s lifecycle. 

2. Does every clinical trial require a medical monitor?

Every clinical trial needs an appropriate safety monitoring approach, but not every study requires the same structure. The level of oversight should reflect the trial’s risks, size, design and complexity. A small, low-risk study may use an independent medical monitor, while a larger or higher-risk trial may also require a safety committee or DSMB. 

3. What qualifications should a clinical trial medical monitor have?

A medical monitor is generally a physician or another suitably qualified medical expert with relevant experience in the condition, treatment or therapeutic area being studied. For independent monitoring roles, the individual should also be free from conflicts that could affect their safety recommendations. 

4. What is the difference between a medical monitor and a clinical research associate?

A clinical research associate primarily oversees trial progress, site activities, records and compliance with the protocol and Good Clinical Practice. A medical monitor applies clinical judgement to participant eligibility, adverse events, laboratory findings, benefit-risk assessments and emerging safety concerns. The two roles collaborate but have different responsibilities. 

5. How often should clinical trial safety data be reviewed?

There is no single review frequency suitable for every clinical trial. The schedule should be defined in the data and safety monitoring plan according to the trial’s risk level and design. Serious or unexpected safety concerns may need immediate assessment, while cumulative data may be examined through scheduled periodic reviews. 

6. Can medical monitoring services be outsourced to a CRO?

Yes. A sponsor may transfer specific clinical trial responsibilities to a contract research organisation, including relevant monitoring and safety activities. The transferred duties should be clearly documented in writing. However, outsourcing does not remove the sponsor’s overall responsibility for the quality and integrity of the trial data.
